City :		Enter your company's city name.
State / Province :		Select your company's state or province if USA or Canada.
Country :		Select your country.
Postal Code :		Enter your postal code.
Toll Free Phone # :		Enter your company's 800 number in the format 800-999-8888.
Direct Phone # :		Enter the phone number you want readers to call you at.
Fax # :		Enter your company's fax number.
Email Address :		Enter the email address you want readers to contact you at.
Web Address :		Enter your company's home page URL without the "http://" code.
